2025 H.O.M.E. Awards

Every day, across South Carolina, countless individuals dedicate themselves to providing care, comfort, and compassion to those who need it most. These are the quiet heroes—physicians, professionals, in-home aides, and family caregivers—who go above and beyond, often without recognition. Their work is not just a career or a

responsibility, but a calling rooted in service, love, and humanity.

The H.O.M.E. (Honoring Outstanding Merit and Excellence) Awards were established to recognize individuals who embody the very best of home care, home health, and hospice. This is your chance to share their stories, to lift them up, and to let their light shine in front of peers and colleagues.

To submit a nomination form for an eligible individual, select a category below and complete the form, ensuring you do not skip any questions. ​​​

 

GUIDELINES: Be sure to fully answer each question (answers are part of the scoring criteria). - Do not use the name of the nominee or any identifying information (such as agency name) on your narrative or in your answers. - The only place the nominee's name and agency should appear is in the nominee first/last name fields at the top of this form. - The Association's PR Committee will receive the narrative portion of the nominations only so that the awards selection process will remain anonymous. - Simply select your nomination category by clicking one of the buttons below and fill out the nomination form.
